For the last 30 years neurotoxins have been used for a wide range of ailments, from Cerebral Palsy in children to twitching eyes and strabismus. It was from treating strabismus in the eye area that Medical pioneers, Dr. Jean and Alistair Carruthers, discovered that a side effect of the drug was elimination of wrinkles. What a discovery!
Neurotoxins are now the gold standard treatment in elimination of wrinkles caused from excessive and repeated facial expressions and muscle movement. It is a safe and effective treatment with no downtime used by millions of people worldwide.
HOW DOES IT WORK?
Anti-wrinkle injections work by blocking nerve impulses needed for muscle contraction. Once the muscle is relaxed, the muscle cannot be contracted and the undesirable facial expression is reduced or eliminated.
HOW LONG DOES IT TAKE TO WORK?
Normally takes 2-3 days to work with optimal results seen after 7-10 days. It usually lasts between 3 – 4 months.
WHICH AREAS CAN BE TREATED?
For the treatment of wrinkles, the most common areas to treat on the face are the frown lines (between the eyes), the crows feet (around the eyes) and the horizontal lines on the brow.
Anti-wrinkle injections can also be used in the lower face. These areas include:
CHIN: Occasionally the chin is quite bulky or can have an orange peel appearance. This is caused by overactive muscle in this area. Neurotoxins can be used to relax the muscle, smooth out the skin and lessen a prominent chin.
JAW: A square jaw on a female can appear very masculine, this bulky appearance of the masseter muscle on either side of the jaw can be due to genetics or teeth grinding. By using a neurotoxin in this area, the face can be slimmed down and a more oval face shape can be achieved. For patients who are chronic teeth grinders, it is a good treatment to avoid wearing away the teeth and avoid headaches due to excessive grinding.
DOWNWARD SLOPING MOUTH: As we age the muscles around the mouth can cause the mouth to be pulled down at the edges making for a sad looking face. By using neurotoxin in this area we can relax these overactive muscles and create a more youthful looking mouth.
UPPER LIP: Vertical lines on the top lip (smokers lines) are most common in women and can be very ageing. Talking, smoking and drinking all cause us to purse our lips contributing to the upper lip lines or lipstick bleeds. Neurotoxins can be used in this area to alleviate this problem.
NECK: The platysmal muscle covers the neck. As we age, vertical bands can appear on the neck either at rest or as we talk. Relaxing this muscle can help with jowling in the lower face and make the platysmal bands disappear.
WILL IT BE PAINFUL?
The injections are relatively painless and are over very quickly. Most people can tolerate the injections comfortably. A very fine needle is used to ensure maximal comfort. Ice can be used if the patient prefers.
ARE THERE ANY SIDE EFFECTS?
There are side effects as with any medical treatment, however they are minimal. There is a risk of bruising, infection, headache, double vision and eyelid and brow ptosis (all of these are very rare and temporary).
I DON’T WANT TO HAVE THE FROZEN LOOK, I JUST WANT TO BE REJUVENATED? IS THAT POSSIBLE?
Yes definitely. The aim of our treatment is to give you the most natural look possible. We want your friends and family to say you look refreshed not overdone! This is why at Privée we formulate a specialised treatment plan tailored to you, taking into consideration the look you want to achieve.
In the last few years we have seen a shift in the use of dermal fillers. When they first came onto the market, injectors concentrated on individual lines and wrinkles, nasolabial folds in particular. Over time it became apparent that filling a single wrinkle did not provide a beneficial outcome for the face as a whole. It may have reduced the individual wrinkle itself but the patient was often left with the same tired, ageing face.
The focus has now shifted from wrinkle filling to a process called pan facial rejuvenation. This entails using filler to correct and support the underlying facial structures and to restore lost volume resulting in soft youthful curves. The focus is now about fixing the broader problem instead of an individual symptom.
As we age our face changes in many ways. The underlying bony facial structure changes and we develop bone loss in certain areas of the face which in turn results in an aged appearance.
At the same time we also begin to lose fat from our faces. The most common areas where this occurs is in the cheek and mouth area.
Because we no longer have the fat plumping out the tissues, the skin begins to droop leading to the development of jowls and a tired sunken face. Our once elastic and perfectly plumped skin is now drooping and in desperate need of rejuvenation.
Cue our hero to the rescue – dermal fillers!
At Privée Cosmetic Clinic we only use the best dermal fillers to ensure the most natural and long lasting results. All our dermal fillers also come with local anaesthetic which means it is a relatively painless experience overall.
WHICH AREAS CAN BE FILLED?
Filler can be used almost anywhere in the face to correct volume loss and fill lines. This includes cheeks, temples, lips, lip lines, marionette lines, nasolabial folds and jawline. Filler can also be used in the hands.
HOW LONG DOES IT LAST?
This depends on which filler is used. Generally the filler can last anywhere between 6 months- 2 years. For example on larger areas such as the cheeks a highly robust filler is used which lasts about 2 years whereas a lighter filler for an area such as the lips will last approximately 6-9 months.
WILL IT BE PAINFUL?
The filler contains a local anaesthetic so after the first few injections is relatively painless. Ice or topical anaesthetic can also be used if the patient wishes. A dental block can also be used when treating the lips. This makes the whole mouth completely numb.
IS IT SAFE?
Yes. The filler is made from a sugar that naturally occurs in the matrix of our skin. It is this sugar that gives our skin its volume. Allergic reaction is extremely rare.
WHAT ARE THE OTHER SIDE EFFECTS?
As with any medical treatment there are some general side effects -bruising, swelling, redness and infection may occur. Very rare is necrosis of the skin, migration of the product to another area. Lumps may be felt under the skin however these usually resolve within a week or two after the filler has settled.
IS IT REVERSIBLE?
Yes. If for some reason you don’t like the filler then it can be dissolved.
IS THERE ANYTHING I CAN DO TO PREVENT BRUISING?
No matter how careful and gentle the injector, sometimes bruising is hard to avoid. The face is highly vascular. There are a few things you can do beforehand to minimise the chance of bruising.
Avoid drinking alcohol before and after the treatment.
Avoid taking aspirin, Vitamin E, Gingko Biloba, Anti-inflammatories (Nurofen) and fish oil for 3 days before the treatment. Seek advice from your dr before ceasing blood thinners such as warfarin or Plavix.
A herbal preparation called arnica which is available from most pharmacies can be taken before and after the treatment. It is available in tablet, sublingual and topical cream preparations.
I HAVE FIXED THE WRINKLES BUT WHAT CAN I DO ABOUT MY OVERALL COMPLEXION?
Sun damaged skin (photo-ageing) can also contribute to looking old beyond your years. Anti wrinkle treatments and fillers are great for addressing wrinkles and volume loss however skin texture is another important factor in having a youthful appearance.
Sun damage caused by UV radiation from the sun, free radical damage from pollution, smoking, ill health and dehydration can all take their toll on the skins complexion. The cell turnover of our skin slows down with age. This can cause our skin to appear dull and lifeless. By using a chemical peel or skin needling we can slough off the top layer of dead skin cells to reveal a brightened glowing skin underneath.

WHAT EXACTLY IS HYPERHIDROSIS?
HYPERHIDROSIS IS THE TERM GIVEN TO THE PROBLEM OF EXCESSIVE SWEATING. UP TO 5% OF THE POPULATION SUFFER FROM THIS MEDICAL PROBLEM.
Sweating is a normal physiological phenomenon, which helps regulate body temperature by cooling the skin through evaporation. The brain detects a change in body temperature and sends a signal to the glands to produce sweat. In a person with hyperhidrosis the brain sends too many signals, which overstimulate the sweat glands leading to excessive sweating. Exercise, stressful situations and hot or spicy food can all stimulate the brain to respond in this manner.
HOW IS IT TREATED?
A grid like pattern is marked out on the underarms and injections of a neurotoxin are then placed superficially in the skin about 2cm apart over the area of the sweat glands. The whole procedure takes about 20mins
HOW LONG DOES THE TREATMENT LAST?
Studies have shown that the median length of time for the treatment to last is 6.7 months. However some people may have a reduction in sweat, which lasts up to 1 year.
ARE THERE ANY SIDE EFFECTS?
AS WITH ANY TREATMENT THERE MAY BE SIDE EFFECTS. MOST ARE UNCOMMON BUT CAN RANGE FROM:
- Pain at the injection site
- Redness and swelling
- Temporary arm weakness
- Perceived increase in sweating in another body area
WHAT ARE THE CONTRA-INDICATIONS?
- Pregnancy or breastfeeding
- Neuromuscular disease
- Certain medications
- Allergy to neurotoxins
